Chronic Care Improvement Program

The Chronic Care Improvement Program (CCIP) is a home visit based program. Every year VIVA Medicare Plus targets a chronic disease to be the focus of the program. The disease is selected based on its prevalence in our membership and the opportunity for quality improvement to improve the member’s health.

The 2011 program focus is on Diabetic members who need to have their diabetic tests (HgbA1c, cholesterol, LDL, renal function, and eye exam) performed. The case manager visits the members during the year to discuss and educate all aspects of diabetes, educate on the need for the tests to be performed and encourage the member to follow up on a regular basis with their physician.
