Chronic Care Improvement Program

The Chronic Care Improvement Program (CCIP) is a home visit based program. Every year VIVA Medicare Plus targets a chronic disease to be the focus of it CCIP. The disease is selected based on its prevalence in our membership and the opportunity for quality improvement.

The 2010 CCIP focus is on Diabetic members with an HgbA1C of 8.0 or greater. The case manager visits the members every other month to discuss and educate all aspects of diabetes, teach the member how to use a glucometer and encourage the member to work toward measurable goals.
